According to Tennessee Highway Patrol, the accident happened at around 7 a.m. when the jeep, driven by Jesus Diaz De Leon, hit a bus containing 19 students and a driver. The accident, which happened at the intersection of Columbia Pike and Kittrell Road, occurred after De Leon did not stop, pulling out in front of the bus and  causing the bus to strike the jeep on the driver’s side. De Leon suffered fatal injuries, while the bus driver and one student sustained injuries.

After the accident, uninjured students were transferred to another bus and taken to Independence High School.

Alcohol was found inside the jeep, but authorities are not sure if De Leon was intoxicated during the crash. The accident is currently being investigated.

State Trooper Todd Logan said the accident happened around 6 p.m. when Melford E. Thomas, 41, of Cookeville, who was heading north, lost control of his vehicle. Thomas’ car rolled over after it crashed into a southbound vehicle driven by 55-year-old Michael S. Madden. Madden and his passenger were reportedly not injured in the crash as they were both wearing seat belts when the crash occurred. Another passenger from Thomas’ vehicle was also reportedly not injured in the crash.

Thomas was reportedly not wearing a seat belt and was pronounced dead when he was brought to Cookeville Regional Medical Center.

Tennessee Highway Patrol (THP) said 69-year-old Leta Fisher died after her vehicle was struck on the driver side by a westbound vehicle driven by Mary Miller. Miller and the two passengers from Fisher’s vehicle, 14 and 17-year-old males, were injured in the crash. A passenger from Miller’s vehicle, Jason Bates, 37, was not injured. Trooper Jack Alderman said occupants of both vehicles involved in the crash were wearing seat belts.

Lt. Brian Lawson of the THP assisted in the investigations. No charges were filed in the fatal crash.

The tragic crash happened on West Andrew Johnson Highway near West Economy Road in Morristown around 7 a.m. According to reports, 73-year-old James Lakin and 27-year-old Brent Michael Bennett, both from Morristown, died in the accident. The accident happened around 5 a.m. on Interstate 40 in Cocke County Tennessee when van driver 35-year-old Kristy Lyn Jones of Toledo was on her way to a family vacation had a head on collision with an SUV. The driver of the SUV was identified as Dan Williams, 39, of Newport Tennessee.

Authorities requested Williams to undergo drug and alcohol test, which left charges pending while awaiting the results.

The 25 year-old driver has been arrested and taken to the Shelby County Jail on charges of driving under the influence, driving with an open container or alcohol, leaving the scene of an accident, driving without insurance, and public intoxication. He is being held on $200,000 bond.

The woman’s death puts Tennessee’s traffic fatalities up to 825, the Commercial Appeal reports. This number is up four percent compared to this time last year, which had 791 deaths.

The facility has voluntarily closed its doors until the Centers for Disease Control and Prevention determine there is no longer a risk to patients. According to News Channel 5, the CDC, Food and Drug Administration, Saint Thomas, and other hospitals are involved in the investigation.

Multiple states have been hit with the meningitis outbreak, leading to the deaths of over a dozen people.

The form of meningitis affecting the patients is not contagious as it is linked to the steroid injections. Meningitis is a serious and potentially fatal disease that affects the brain and nervous system.

Witnesses say they saw the woman put the cart on the escalator, which leads from the store to the parking garage, then watched as the cart fell on top of her. She then fell to the bottom of the escalator.

The woman was transported to a local hospital in serious condition, but she later died due to her injuries. According to WSMV, Harris Teeter has escalators for pedestrian use as well as ramps for shopping carts.

The 48-year-old pleaded guilty to the DUI charge and was sentenced to two years in prison and fined $3,000 for the offense. However, since he had several previous convictions, the court fined him an additional $10,000 for violating probation.

According to Knox News, officers say the man had bloodshot eyes, smelled of alcohol, and slurred his speech when they stopped him. The driver will be eligible for parole after serving less than eight months.

According to News Channel 5, witnesses told police that after the woman drove off from the scene of the accident, she attempted to merge into on-coming traffic. Officers were able to stop her before she caused any more accidents.

The driver refused chemical testing at the scene, forcing officers to take her to Williamson Medical Center to have her blood drawn. She has since been released from jail on bond.
